Where are you from?

Literal

Honorific-origin [topic-は] which-direction [copula-です-question-か]?

A polite way to ask someone's place of origin. ご出身 uses the honorific prefix ご (used with Sino-Japanese words; お is for native words) on 出身 ('origin, birthplace'). どちら is a polite version of どこ ('where') — literally 'which direction' but commonly used to mean 'where' in formal speech. The whole phrasing is markedly polite.
