なさる is the honorific (sonkeigo) form of する — 'to do.' It conjugates irregularly: the ~ます form is なさいます and the ~て form is なさって. Here なさっています is the honorific progressive/state, elevating the listener — standard phrasing from hotel or restaurant staff. Far more respectful than していますか or されていますか.
