Friends [topic-は] few [only-しか] not-exist.

しか~ない ('only, nothing but') requires a negative predicate and emphasizes scarcity — 'there exist only few.' The closely related neutral phrasing 少しいる ('there are a few') flips the tone from lamenting to matter-of-fact: the same number of friends, but a different feeling about it. Unlike だけ (neutral), しか always carries an undertone of 'and it's not enough.'
